Questionnaires/Forms

 

In the referrals screen you can identify Questionnaires/Forms that are New, In Process, or Completed status.

 

1.     You can click the ‘New’ link on the left navigation bar. A table with the new Questionnaires/Forms will be displayed.

a.     The table will show Workflow ID, Questionnaire/Form #, Patient Name, Site #, Facility, Received Date, Status, Type columns.

b.     You can filter the table by entering the last name of the patient in the Filter by Last Name field and clicking the Go button.

c.     You can sort the Questionnaires/Forms by clicking the columns headings to change the order in which the questionnaires/forms are displayed.

d.     You can click the Select button to go to the Workflow ID screen to view Patient Information and to complete the questionnaire.

e.     You can click the numbers links at the bottom of the table to navigate through the available pages that contain New Questionnaires/Forms.

 

2.     You can click the ‘In Process link on the left navigation bar. A table with in process Questionnaires/Forms will be displayed.

a.     The table will show Workflow ID, Questionnaire/Form #, Patient Name, Site #, Facility, Received Date, Status, Type columns.

b.     You can filter the table by entering the last name of the patient in the Filter by Last Name field and clicking the Go button.

c.     You can sort the Questionnaires/Forms by clicking the columns headings to change the order in which the questionnaires/forms are displayed.

d.     You can click the Select button to go to the Workflow ID screen to view Patient Information and to complete the questionnaire.

e.     You can click the View button to display the Questionnaire.

f.      You can click the numbers links at the bottom of the table to navigate through the available pages that contain In Process Questionnaires/Forms..

 

3.     You can click the ‘Completed’ link on the left navigation bar. A table with completed Questionnaires/Forms will be displayed.

a.     The table will show Workflow ID, Questionnaire/Form #, Patient Name, Site #, Facility, Received Date, Status, Type columns.

b.     You can filter the table by entering the last name of the patient in the Filter by Last Name field and clicking the Go button.

c.     You can sort the Questionnaires/Forms by clicking the columns headings to change the order in which the questionnaires/forms are displayed.

d.     You can click the Select button to go to the Workflow ID screen to view Patient Information and to complete the questionnaire.

e.     You can click the View button to display the Questionnaire.

f.      You can click the numbers links at the bottom of the table to navigate through the available pages that contain Completed Questionnaires/Forms.
